How Do I Get Established?
Thinking about becoming a patient at Middle Path Medicine? Here's what you need to know!
Every new patient must fill out a medical history package, accessible below or for pickup in the office, prior to establishing.
Middle Path Medicine is a cash-pay office. We do submit your insurance for lab work, and will need copies of your insurance cards to submit for prescriptions and referrals.
We will arrange lab work for you with the suitable lab covered by your insurance.
We also offer a Super Bill for visits, which you may submit to your insurance for reimbursement. Please note, our office is not contracted with Medicare, therefore you may not submit to Medicare for reimbursement. If you have any questions about insurance coverage or submitting a Super Bill, feel free to reach out to our team for assistance.
If you don't have insurance, we've partnered with our labs to provide affordable cash pricing!

Visits With Dr. Foresman
Dr. Foresman's initial consultations last 90 minutes. Follow-up appointments with Dr. Foresman are typically one hour.Currently, consultations with Dr. Foresman are exclusively offered via Phone or Zoom call.
