Changes You Can Make Today
Did you know there are changes you can make before you even see Dr. Foresman? Are you ready for a change but not sure where to start? Dr. Foresman has written two different articles that have all the information you need to start your journey to health and healing. Read The Foundations of Health for more information on lifestyle habits he recommends as a standard for all patients. He has also developed a Basic Nutritional Protocol, the basis for a successful supplement routine.
How Do I Get Established?
If you are considering becoming a patient at Middle Path Medicine, here are a few things you need to know:
Each patient is required to complete a medical history package (available below or in the office), and to see the Medical Assistant for a Wellness Evaluation before seeing Dr. Foresman (for one and one half hours), or Jessica Joslyn, PA-C (for one hour) for your initial appointment, or before receiving Intravenous Nutrition Therapy.
Follow up visits with Dr. Foresman are one hour long or with Jessica one-half hour long, but you can schedule longer appointments when necessary.
New Patient Forms
The forms below are required prior to your first visit with Dr. Foresman or Jessica C. Joslyn, PA-C. Please take the time to fill out EVERY form. We will contact you to schedule appointments once paperwork is received.
